Up front, what I'd always assumed was just a cap that covers and
protects the actual reg adjuster, is in fact the adjuster itself.
The entire cap is turned to set the output pressure- the two larger holes presumably accomodate a key or wrench of some sort,
in order to turn the cap under pressure- since it's slick and un-knurled, and probably impossible to turn when pressurized.
The center hole is just a safety vent, and the setscrew is the "tourney lock"- once the setscrew is snugged in,
the cap can't be turned in- to raise the pressure- any further.
