Clearly the nut itself needed to be trued up, and here's the technique:
First, we take an appropriate chunk of roundbar out of the junk bin, and turn a short
stud of new threads on the end. Make sure the roundbar you select is big enough that once
you turn the threads, you have a fair "shoulder" to tighten the nut up against.
Use care, and make the threads fit nice and snug. I don't recommend using
things like tailstock die holders or even dies in general, for this.
You want to make the threads as square to the headstock as possible.
